Home Care Company

Caregivers are supervised by Trustiva Health's case managers who provide support for all clients and caregivers in the home.

Our array of professional services provide clients assistance with:

  • Activities of Daily Living (ADL's): bathing, dressing, oral care, toileting, transferring, transporting, eating, cooking, serving meals, running errands, shopping, housework, driving
  • Medication Management
  • Wound Care
  • Coordination of Services for Rehabilitation
  • Companionship
  • Implementing Prescribed Orders & Treatments
  • Maintaining Prescribed Orders & Treatments
  • Respite Care
  • Attending Doctors and Therapy Sessions
  • Transportation to Appointments or Social Events

Trustiva Nursing Case Management (if required):

  • Conduct a medical history and nursing evaluation
  • Monitor vital signs
  • Medication review
  • Medication set up
  • Collect Medical records and HIPPA releases
  • Make recommendations to physicians such as gerontologists, geriatric psychiatrists, PT, OT, Speech, psychotherapy, neuropsychology and hospice when indicated
  • Develop a home exercise program
  • Develop a recreational program
  • Dressing changes
  • Arrange transportation as indicated
  • Research treatment modalities
  • Liaison with all practitioners and family members
  • Support caregiver staff
  • Attend physician appointments with the client

We conduct the following screening processes and practices for every employee to ensure you are getting the utmost in quality, skill and professionalism every time:

  • All caregivers are licensed CNAs or equivalent experience
  • Exceptional skills and experience
  • Criminal background check
  • Reference checks via phone or mail
  • State health screenings as required by law
  • Drug Screening
  • CPR certification

Once screened and approved for hire, each Trustiva Health employee undergoes:

  • Competency verification
  • Supervisory visits for personal care and skilled service cases
  • On-Site orientation prior to placement
  • Employee performance reviews within the first 90 days and then annually after that
